Someone give me some advice here. I want to leave my home for good. I currently live with my grandfather because my other parents are evil and hate me. I want to just pack up and leave. no note or anything. just going. what is the best way to prepare for this. What should I be bringing with me. What is something that I simply must do after I have left. I'm 20 years old.

Hmm. I mean, I've never done something like that before, but I know people who have been in situations where they almost did it.
One thing that's very important is making sure you always have somewhere to be. Be it enough money to sleep at hotels/motels, being able to crash with people you know or a car/van/other vehicle with walls and a roof.
Then there's obviously food, clothing and first aid. There's no point in escaping for a better way just to die while you're search for it.

Money is obviously a part of the essentials as well. You need to have enough money to survive for a long period of time. And, it probably goes without saying, but water is of course an essential. You ain't getting anywhere without it.
Spare clothes so you can get changed whenever you rest and in case of rain or similar.
Then you need to map out routes you can follow and places you can go that have stuff like stores and places you can stay at. Wandering aimlessly won't work.

You should let someone you trust know about it. A close friend or similar that you know won't say anything and that will be there for you in case of emergency. Not having contacts or anyone you can rely on at all can end with you just dying somewhere or having to go back.
Then you need to find out ways to earn money. If paid small jobs for people is a common thing where you live then be prepared to not it, and if not find ways to stay in one location for a week or two for work.

You obviously also need to know how to use the first aid you bring, and yes, you have to bring a first aid kit at the bare minimum.
A new phone and having all your money in cash instead of on card is also important if you want to reduce the chance of getting tracked.
If you can manage all that, then the next step is being prepared for how harsh and rough it's going to be. Every day is going to be exhausting and mentally draining.
If you can manage all of that, then you're ready.
